A short questionnaire for adults living with obesity, about a new and experimental approach to treatment. There are no right answers. We want your honest opinion — including if the idea doesn't appeal to you, or if it worries you.

Who is asking
This questionnaire is run by Sella Therapies, a company developing an experimental treatment for severe obesity. We want to understand what people living with obesity actually experience — what has helped, what hasn't, and what is missing. Your answers will inform how we develop our products and services.
What we ask about
A few questions about your experience: your age range, roughly where your weight sits, whether you have used a GLP-1 medicine such as Wegovy or Zepbound, and whether weight-loss surgery has come up for you. Then we describe the idea and ask what you make of it.
At the very end we point to ResearchMatch, an independent volunteer registry for health research funded by the National Institutes of Health. Joining it is optional, entirely separate from your answers here, and we have no way of knowing whether you did.
What happens to your answers
We don't ask for your name or contact details. We don't record your IP address or place tracking cookies. We can't identify you, and we can't link your answers back to you.
Please don't include your name, the names of your doctors or family members, your medical record number, or anything else that could identify you or someone else — including in the open-text boxes. We keep responses for 24 months and then delete them.
